New Forms Fee

Attention parents and guardians

Effective 9/1/10, there will be a $5.00 fee for completion of ALL daycare, camp, school, or sports physical forms not presented at the time of your child's well child or physical exam appointment.

Payment of cash/check must be given at drop off and before form is completed.  For faxed and mailed-in forms, payment must be given at pick-up.

What is Influenza, the "flu" and H1N1 or "swine flu?"

The flu is a contagious respiratory illness caused by influenza viruses. It can cause mild to severe illness, and at times can lead to death. The best way to prevent seasonal flu is by getting a seasonal flu vaccination each year.

Every year in the United States, on average:
5% to 20% of the population gets the flu; more than 200,000 people are hospitalized from flu-related complications; and about 36,000 people die from flu-related causes.
